Practical support for everyday living, confidence, and community connection.
At Health & Community Services (HCS), we’re committed to helping NDIS participants build independence, confidence, and a sense of belonging through personalised, high-quality support.
Our core supports are designed to help you manage daily life with dignity and ease — whether that’s at home, out in the community, or travelling to appointments. We work alongside you to develop routines, build skills, and create opportunities that empower you to live life on your own terms.
Assisted travel and transport
Getting to appointments, social events, or community activities should be simple and stress-free. Our transport services provide safe, reliable, and flexible options that match your lifestyle and level of independence. We can support you by:

- Assisting with mobility aids or equipment during your journey
- Arranging private or supported transport, including taxis or travel with a support worker’s vehicle
- Ensuring safe travel to appointments, community programs, or social activities
- Encouraging independence and familiarity with routes, timetables, and navigation
