At Health & Community Services (HCS), our support coordination team helps participants make the most of their NDIS plans by connecting them to the right supports, coordinating services, and ensuring they have the knowledge and confidence to make informed decisions.

Our support coordination is person-centred, empowering, and collaborative. We focus on you — your abilities, aspirations, and what truly matters in your day-to-day life. Our experienced support coordinators build genuine relationships based on trust, transparency, and respect — and help participants build the confidence and skills to self-direct their supports over time.

Plan implementation

• Explaining your NDIS plan and funding categories in plain language
• Helping you understand budgets, service bookings, and reporting
• Coordinating supports so they complement one another effectively

Connecting with providers

• Linking you with a trusted network of NDIS-approved service providers
• Ensuring your chosen services align with your goals, preferences, and cultural needs
• Reviewing service agreements and supporting clear communication between providers

Building capacity and independence

• Providing tools and training to help you develop self-management skills
• Coaching you to make informed decisions and advocate for your own needs
• Encouraging confidence in budgeting, scheduling, and progress tracking

Resolving issues

• Acting on your behalf to resolve concerns with service quality, delivery, or funding
• Supporting you during plan reviews or reassessments
• Liaising with the NDIA, plan managers, and service providers so your voice is heard
