Professional, compassionate care — delivered with dignity, respect, and clinical excellence.

At Health & Community Services (HCS), we understand that quality nursing care is essential for maintaining health, confidence, and independence — especially for individuals with complex medical or disability-related needs. Our team of qualified registered and enrolled nurses provides professional, person-centred support across home, community, and Supported Independent Living (SIL) environments.

Core nursing support

Every participant receives a personalised care plan, developed in collaboration with healthcare providers, families, and allied health teams to ensure continuity and safety. Our key nursing supports include:

  • Medication management: safe, accurate administration and monitoring of prescribed medications, with education for participants and families
  • Wound care: comprehensive assessment, treatment, and monitoring to prevent infection and support healing
  • Chronic disease management: support for conditions such as diabetes, hypertension, or respiratory illness
  • Pain management: evidence-based strategies to improve comfort and mobility
  • Continence management: sensitive, dignified assessments, aids, and routines

Home nursing support

Many people feel most comfortable receiving care in their own home. Our home nursing brings clinical expertise directly to where you live:

  • Skilled nursing visits: health assessments, wound reviews, injections, dressings, and chronic condition management
  • 24/7 nursing care: around-the-clock clinical support and on-call availability for participants with high medical needs
  • Respite nursing care: temporary care to give family caregivers time to rest and recharge

Specialised mental health nursing

As part of our multidisciplinary approach, our credentialed mental health nurses deliver recovery-oriented support, including administration of long-acting injectable medications, mental state monitoring and crisis intervention, relapse prevention and wellbeing planning, and collaboration with psychiatrists, GPs, and mental health services.
