Empowering independence through care, comfort, and community.

At Health & Community Services (HCS), we’re committed to providing exceptional Supported Independent Living (SIL) accommodation for NDIS participants across Victoria. Our SIL homes are designed to empower individuals to live as independently as possible — while receiving the right level of support to feel safe, confident, and connected.

We believe that independence looks different for everyone. Whether you need minimal assistance or 24/7 care, we’ll tailor your supports so that you can live life your way, in a home that feels truly your own.

What is Supported Independent Living?

Supported Independent Living (SIL) is an NDIS-funded support designed to help people with disabilities live in shared or individual accommodation while receiving assistance with everyday activities. SIL is ideal for people who want to maintain their independence but require regular support with personal care, daily routines, or household management. Our model is built around empowerment, dignity, and choice.

Modern and accessible homes

Each property is modern, clean, and thoughtfully maintained. Features may include wheelchair access and mobility-friendly design, wide doorways and accessible bathrooms, emergency call systems and safety equipment, air conditioning and heating, and outdoor spaces for relaxation and socialising.

24/7 support from dedicated staff

Participants receive round-the-clock support from our trained and compassionate team of support workers, nurses, and allied health professionals. Support may include personal care and daily living assistance, medication management and health monitoring, meal preparation and household chores, emotional support and companionship, and night-time supervision and crisis response.

Individualised support plans

Each SIL resident receives an individualised support plan, co-designed with them and their support network and reviewed regularly. Supports may include daily living skills development, social and emotional wellbeing support, participation in therapy and health appointments, and assistance with building community connections.

Social, recreational and community activities

Our SIL homes regularly host group activities, social outings, and skill-building programs — from group cooking and movie nights to local outings, gardening and art programs, and community volunteering — helping participants build confidence, improve communication, and feel a sense of belonging.
